To download the files to your computer: right click on the file name and then—depending on your browser—choose either "Save target as..." or "Save link as...".

How to take advantage of fillable PDF forms

For your convenience, we have created many of the pdf forms with fillable fields. To take advantage of this feature, open them with Adobe Reader. Adobe Reader can be downloaded for free at get.adobe.com/reader/.

Contact the office if you are experiencing a problem with any form. We will need to know your operating system, Adobe Reader version, the name of the form, and the type of problem you are experiencing.

How to take advantage of jpeg images

Jpegs can be downloaded and inserted into your program of choice, e.g., Microsoft Word. Text and a logo can then be added to personalize.
